Illawarra Specialist Plastic and Aesthetic Surgery  (ISPA) Clinic is the realisation of a goal to provide an extensive and comprehensive specialised plastic, aesthetic and reconstructive service to our home community in the Illawarra where we have chosen to raise our family, surrounded by family.

Updated Guidelines: Medical Board of Australia (MBA) – Effective July 1, 2023

The MBA has introduced new guidelines for cosmetic procedures, effective July 1, 2023. Key changes include:

  • GP Referral: Required for cosmetic surgery.
  • Advertising Rules: Stricter regulations.
  • Patient Selection: Strengthened guidelines to assess psychological suitability, including screening for conditions like body dysmorphic disorder (BDD). Patients with significant psychological issues will be offered referral to independent professionals

Background

Dr Knight is a Specialist Plastic Surgeon and a fellow of the Royal Australasian College of Surgeons (FRACS). He offers an extensive service covering general plastic, reconstructive and aesthetic surgery, as well as having obtained super speciality qualifications in craniomaxillofacial and paediatric surgery. He completed his undergraduate training in South Africa at the world-renowned Baragwanath Hospital before undertaking further studies in the Europe (UK, Scotland, Italy, France and Sweden) where he also became a member of the Royal College of Surgeons of Edinburgh. He undertook five years of translational lab based research working on the angiolymphatic reconstitutive properties of umbilical cord blood stem cells in surgery at the Prince of Wales, Sydney, Australia. His specialist training was completed in Western Australia before fellowships in Brisbane, Sydney and the United States where he worked with colleagues from Harvard, Princeton, Stanford and Washington universities over 20 years of training.

He holds conjoint lecture positions at various Australian universities where has received numerous awards and has a keen interest in translational molecular and clinical research which has been published and presented internationally as well as various book chapters relating to craniofacial trauma. As part of his ongoing commitment to continued medical education Dr Knight spent a considerable amount of time with FACIAL Team in Spain consolidating his skill set with respect to gender diverse patients and their specific needs. He also travels to the Philippines for humanitarian work involving cleft lip and palate work. Dr Knight is a current Specialist Consultant Plastic and Reconstructive Surgeon at Wollongong Public Hospital where is the Director of Training for Plastic Registrar Trainee sand holds a conjoint associate lecturer position with the University of Wollongong. He is based at ISPA Clinic on Crown St in Wollongong however does consult one day/ week on Macquarie Street in Sydney.

Facial Surgery

Rhytidectomy (Face lift/neck lift)

Rhytidectomy surgery is a number of procedures designed to address wrinkling in the facial and neck region. This can be due to either congenital issues, following cancer ablative therapy, facial nerve injury or as a consequence of age. This surgery aims to address normal functional goals of the face e.g. eyelid integrity as well as providing an improved aesthetic outcome. The approach to this surgery is to try to correct and improve changes from the facial skeleton to the skin and all structures between those two fields. This involves skeletal work, fat grafting and muscle augmentation, facial muscle suspension and tightening, liposuction, followed by post surgery skin surface rejuvenation with a variety of post ablative and non-ablative options. Concerns such as hollowing and loss of cheek volume, descent of the face, jowls, deep wrinkles, excess skin and fullness below the neck are improved along with skin tightness, texture and colour. This surgery is often performed through incisions along the hairline and below the chin.

Rhinoplasty/Septoplasty

Rhinoplasty, or nose surgery, is a common procedure designed to improve the aesthetic appearance, position, proportion, projection, rotation and/or function of the nose. It may also be performed in conjunction with a septoplasty which corrects the position of the septum (wall) that separates the two sides of your nose. These issues may be due to genetic, congenital, traumatic or pathological effects on your nose.

Blephroplasty (Eyelid surgery)

Blepharoplasty, or eyelid surgery, is a procedures designed to address issue relating to your eyelids – both upper and lower. These issues can be related to excess skin (blepharochalasia), excess or lack of fat (festooning, ptosis or volume loss), loss of muscle/ suspensory ligament integrity (ectropion) or lack of eyelid support. Age, weight changes, allergies and facial issues can lead to changes in eyelid aesthetics as well as irritation of the eyelashes, globe or conjunctiva or vision limitations. Blepharoplasty aims to address these concerns and often involves removal of excess skin, resuspension of muscle, elimination of fat and tightening of suspensory ligaments to restore periperhal vision, eyelid integrity and reduce puffiness and irritation while also improving the appearance of the eyes.

Otoplasty

Otoplasty surgery is ear corrective surgery. The most commonly performed version of this is correction of prominent ears also known as “bat ears”. This surgery corrects the angle of the ear relative to the skull base, insets the ear base and improves various folds within the ear to try and mimic an ideal ear shape that is less prominent.  When this is done before the age of 18, this surgery attracts a medicare rebate; thereafter it is described as cosmetic. This is often performed in children and our staff therefore have current and compliant Working with Children Checks.

Breast surgery

Breast Augmentation

Breast augmentation, when performed by an experienced accredited specialist plastic surgeon, can enhance smaller breasts, accentuate existing breast tissue, symmetrise breasts if one breast is larger than the other or totally change the shape and position of one’s breasts if there are structural issues relating to normal breast development e.g. Poland syndrome. Adequate assessment and implant selection is paramount to give you your chosen result while providing you with expert advice to reduce complications, unsatisfactory aesthetic results and long term issues.

Mastopexy

A breast lift is also called a mastopexy. This roughly translated means “masto” (breasts) and “pexy” (to lift). It is designed to recreate the appearance of firm and more youthful breasts using your own tissue and without the use of implants. Often women do not want implants placed inside their bodies and this is a good alternative especially if there is not a lot of weight involved. Whether the sagging of ones breasts is from childbirth and breastfeeding, changes in weight, gravity, or the natural aging process, this occurs due to the skin losing its elasticity and firmness, a stretching of the normal breast's suspensory ligaments and a general loss in volume which all lead to a condition known as ptosis.

Mammoplasty Reduction/Breast Reduction

Women who seek a mammoplasty reduction generally have significantly enlarged breasts that they describe as heavy and cumbersome and effect their physical, psychological and emotional wellbeing. Enlarged breasts can also affect everyday normal activities of daily living, work, social and sporting activities and lead to pain, discomfort, skin irritation, shoulder bra indentations and neck and shoulder pain.

Breast Implant Removal/Replace

Breast implants may experience issue with regard to abnormal positioning, migration, contractures or rupture. We offer an extensive service for the removal of breasts implants and capsular resection. Some instances we are able to replace these implants or perform a mastopexy to improve the altered breasts.

Gynaecomastia correction

Gynaecomastia is the presence of an abnormally large amount of breast tissue within a male. This can be unilateral or bilateral. It is often accompanied by pain and social embarrassment. The abnormal breasts tissue can result in sagging of the male breasts/chest, puffy nipples as well as stretching and darkening of the areola. Common associations related to this breasts tissue excess include puberty, weight gain, genetic/hereditary conditions, hormonal changes (normal and abnormal), prescription medications side effects or the use of unregulated (anabolic steroids) or recreational drugs (marijuana).

Body Contouring Surgery

Abdominoplasty

Abdominoplasty surgery is colloquially known as a ‘tummy tuck’. This surgery is usually performed on patients who take issue with excess abdominal skin, excess abdominal fat, weak core muscles or all of the above combined. These changes may be as a result of pregnancy, weight changes or the aging process. Correction of these issues often involves surgical removal of the excess skin and subcutaneous fat while also tightening the core abdominal muscles to flatten the abdomen and restore the integrity of the front wall of the abdomen. There are many derivatives of an abdominoplasty (mini, radical, fleur-de-lis, corset) and the type and technique are usually discussed and determined through detailed history taking, examination and collaboration with your surgeon.

Body lift

A body lift is usually reserved for patients who have experienced a significant amount of weight loss and who have been left with excess skin and very little suspensory integrity across their lower body circumferentially. This procedure aims to remove the excess skin and subcutaneous fat, while incorporating an abdominoplasty into resuspending the front, side and posterior thighs and buttocks. There may also be need to address additional skin around the thoracic region which may be required to correct excess skin in your flanks (lower back) and thorax (chest). There are many derivatives of this procedure (corset, circumferential corset, partial corset, fleur-de-lis, high lateral suspension) and type and technique are usually discussed and determined through detailed history taking, examination and collaboration with your surgeon. This procedure aims to improve functional, hygiene and aesthetic concerns related to the excess around the mid-section of the body circumferentially.

Brachioplasty (arm lift) and thigh lift

A thigh lift and arm lift (brachioplasty) are usually performed within the context of significant weight loss to address the excess skin and subcutaneous tissue that may occur. This procedure aims to remove the excess using a combination of liposuction with surgical excision followed by compression and drainage to alleviate functional and aesthetic concerns.

Gender Affirming Surgery

Services offered

Gender affirming surgery is a collective of procedures specifically catering for the LGTBQI community. We are a trans and gender diverse practice and offer various procedures. TOP surgery refers to all procedures related to affirmation of gender diverse ideals within the chest area. This includes breast augmentation, mastectomy and nipple repositioning and other combinations. We also offer facial feminisation surgery which encompasses brow and forehead contouring, brow lift, orbital aperture opening, canthopexy, blepharoplasty, malar augmentation, jaw contouring and chondrolaryngoplasty.  We also offer both functional and aesthetic septorhinoplasty surgery which is often used to also feminise the nose. 

Dr Knight does not perform BOTTOM surgery but we are part of an extensive network and would be happy to assist with facilitating contacts.

Hand Surgery

Services offered

Our practice covers a broad range of hand surgery services. These services cover hand skin cancer removal and reconstruction all the way through to complex functional reconstruction following trauma, work place accidents as well as age/ repetitive use inflammation, infection and structural compromise. These services include carpal tunnel release, cubital tunnel release, trigger finger, Dupuytrens disease, soft tissue and bone tumour resection, and all injuries such as fractures, avulsions, soft tissue loss, lacerations, nerve injuries, burns, tendon injures and ligament/ joint compromise.

Skin Cancer Surgery

Services offered

We perform many procedures related to the diagnosis, evaluation and removal of all forms of skin cancer and other skin related complaints. We provide a very specialised set of skills usually required to deal with skin cancers in aesthetically sensitive and difficult to access or correct areas. This can be anywhere on the body but we are often asked to assist with removal and reconstruction of surgery on the face and neck region as well as with larger and more difficult areas. We endeavour to primarily remove the cancer while preserving as much of the aesthetic and functional integrity of the body and this often involves detailed discussion and face to face consultation. Our surgeons are Specialist Plastic and Reconstructive Surgeon’s who are Fellows of the Royal Australian College of Surgeon’s (FRACS(Plast)) and are fully trained and experienced in the removal of Non-Melanoma skin cancers (NMSC), (Basal Cell Carcinomas (BCC), Squamous Cell Carcinoma (SCC)) and Melonoma (MSC) skin cancer.
